My daughter Nathalie has had bad knee pain on occasion. On some rides or races she is fine, but on others it is nasty, and always the same leg. This has happened for the past 3 or 4 years (she is now 15). She describes the pain as around the knee cap, sometimes behind it, sometimes below (which made us think it was growth related, but she has not grown at all in 12 months), sometimes more general pain. If it happens, it comes on during the ride, not after. She has seen a sport doc recently, who commented on excessive Q angle on that leg, but did not seem to have much cycling knowledge. We have tried changing shoes, pedals, seats, bike and cleat fit, all without a "cure". At times she has complained more when on her road bike, but other periods she complains when riding off road. So far in the fall race series, she had big pain twice, mild pain once, and one race without any problems. Any suggestions? Thanks.

Have your daughter evaluated by an Orthopedic doctor. She may be having tendon problems, cartiledge problems or ligament problems with the knee. If there is nothing wrong structurally with the knee, it may be something simple that may require some readjustment on the bike set-up. Pain in the front of the knee could be due to the seat being too low, seat too far forward, too much big gear pushing, climbing too much (is the pain worse after the courses with tougher, longer climbs?) or cranks that are too long. If one of these factors may be in play, changes in the bike set-up or riding style may be all that needs to be done.
